Enrollment Forms & Tuition Payment - Ministry Full-Time (Grades 6-12)

Thank you for registering with Calvary Preparatory Academy. This next step is to officially enroll and initiate tuition payments. Please be aware that completing this transaction will enact charges to your card. If you are choosing a recurring installment plan, the plan will automatically recur on the same date each month as the original transaction. NOTE: The school does not initiate these transactions. We only receive them according to the schedule you, the customer, select in this form. Any requests for subsequent changes to the tuition payment you select below will need to be initiated by you, the customer. To change the date, or postpone payments, please contact the school after signing up. By selecting a recurring installment plan, you are agreeing that subsequent transactions will occur according to the plan you select below.

All credit card payments will be charged an additional 3% to cover credit card processing fees. If you are paying in full, or by semester, and would like to avoid the processing fees, the school will gladly receive payment by check, with a preference for automated bill pay. Media Release

As an online school, our online presence is one of the key ways we get the word out about all the wonderful things going on here at CPA! We are so proud of the many accomplishments of our students and staff, and often these accomplishments warrant being highlighted by the school to acknowledge these achievements, to chronicle school activities and promote public awareness of the school. Our student-led Newsletter Team documents school happenings each month in our school newsletter, which is sent out to students and their families via email, as well as published on the school blog located on the school's website. Additionally, from time to time, our students like to share photos about themselves, the ministries they are involved with, their extracurricular sports and activities, etc. Faculty and staff will, from time to time, use still photography or videography in informational newsletters, school brochures, class pictures, social media posts, school blog, yearbooks, broadcasting of CPA graduation ceremony and other printed and/or online materials to achieve those goals. While some are comfortable with social media, etc., we recognize that some families prefer their privacy. With that in mind, please indicate your preference with regard to your child’s likeness being used by Calvary Preparatory Academy for the purpose of positive publicity about school activities or student achievement:

Transcripts

STUDENT TRANSCRIPTS: Although not immediately required for enrollment, it is strongly recommended that you provide transcripts and/or grade reports to the school as soon as possible to ensure proper academic placement.

For all new 6th and 7th grade students, please upload a copy of your student's previous year's grade reports or transcripts. For all new 8th through 12th grade students, please upload a copy of your transcripts. These copies will be considered unofficial and used for placement purposes. Students intending to graduate with CPA should also request official copies to be mailed to our school.

Additionally, if there are any special requests or academic accommodations that you would desire the school to consider for approval, please be sure to note them at this time in the "Additional Comments" section below. All documents and information will be reviewed by Mrs. Paynter, our Assistant Principal. PLEASE NOTE: Mrs. Paynter will not be able to assign classes until this information is provided.
